Selective laser trabeculoplasty: first clinical results 11/02/98

Purpose: Selective laser trabeculoplasty (SLT) is a new technique developed to selectively target pigmented trabecular meshwork. We investigated the safety and efficacy of this technique to lower intraocular pressure (IOP).

Methods: Fifty nine eyes with uncontrolled open angle glaucoma (OAG) with prior medical therapy were treated with the Coherent Selecta 7000 TM (Coherent Inc., Palo Alto, CA), a frequency doubled Q switched Nd:Yag laser. Approximately 110 laser spots were applied over 360° of the trabecular meshwork. Efficacy was determined 5 weeks, 1 month, 3 months, 6 months and one year after the procedure.

Results: The average preoperative IOP was 25.05mmHg .Mean post operative IOP (at one year) was 15.76mmHg. Mean percentage reduction was 32.43% at 3 months after operation, 32.51% at six months and 33.51% at one year. Transient post operative pressure spike of 5mmHg or greater resolved in 24 hours was observed in 5 eyes.

Conclusion: SLT appears to be an effective and safe technique for lowering IOP and could be an alternative to Argon laser trabeculoplasty.

Methods : Twenty five eyes with uncontrolled open angle glaucoma (OAG) with prior medical therapy were treated with the Coherent Selecta 7000TM (Coherent Inc , Palo Alto, CA), a frequency doubled Q switched Nd:Yag laser. 9 eyes (group 1) were considered with an angle pigmentation grade at 3 or 4 and 16 with a pigmentation at 1 or 2 (group 2 ). Approximately 110 laser spots were applied over 360° of the TM. Efficacy was determined 5 weeks after the procedure.

Results . The average preoperative IOP was 24.56 mm Hg .The mean post operative IOP (at 5 weeks) was 16.44 (-31.63 %).The mean percentage reduction was 31.1 in group 2 and 32.6 in group 1. Transient post operative pressure spike (< 10 mm Hg) resolved in 24 hours was observed in 2 eyes.

Conclusion: SLT appears to be and effective and safe technique for lowering IOP and could be an alternative to Argon laser trabeculoplasty.
